Dear Valued Patient,

We are writing to inform you that, effective January 1, 2025, Dr. Howard Lifland will be retiring from Columbia Nephrology Associates, P.A., after 16 years of dedicated service. Dr. Lifland has been an integral part of our team, and while he will be greatly missed, we wish him all the best in his future endeavors.

At Columbia Nephrology, we remain committed to providing the highest standard of care and ensuring a smooth transition for all of Dr. Lifland’s patients. Our practice is staffed by some of the most knowledgeable and compassionate physicians in the state, and we are confident that you will continue to receive exceptional care. Your medical records will remain securely on file with Columbia Nephrology. If you choose another physician within our practice, no action is required regarding your records. However, if you decide to transfer your care outside of our practice, we will need your written authorization to release your records, as we are legally required to obtain consent before sharing any medical information.

Regular Patients: To ensure continuity in your care, if you have a preferred provider or clinic location that you would like to transfer to, please contact our office at 803-252-9907 (press 2 for appointments). If you do not have a preference or do not contact us with a preference, we will assign you to see one of our experienced nephrologists, and you will receive an appointment card in the mail.

Transplant Patients: With this transition, all transplant patients will now be seen at our Columbia office, located at:

121 Park Central Drive, Suite 200 Columbia, SC 29203

We are fortunate to have two excellent transplant nephrologists, Dr. Manish Kadian and Dr. Helga Vamenta-Morris, who will continue to manage your transplant needs. If you have a preference between these two physicians, please call us at 803-252-9907 (press 2 for appointments), and we will do our best to accommodate your request. Otherwise, we will assign you to one of these doctors. You will also continue to be seen by our transplant nurse practitioners on a rotating basis. In addition, you will still have the option to have your labs drawn at either our Lexington/West Columbia office or our Columbia office, whichever is most convenient for you.

We understand that this is a significant transition, and we are here to support you every step of the way. If you have any questions or need further assistance, please do not hesitate to contact us.
